Public Records Request
Pursuant to Chapter 119, Florida Statutes, requests for public records may be submitted in person, by mail, online submission form, email or telephonically. A public records request does not have to be in writing as a prerequisite for completing a public records request. Please feel free to contact the Custodian of Records (Jeff Pawul, Fire Chief) in writing at info@captivafire.com, by phone at 239-472-9494, or by completing the request form below.
Please note, if you wish to use the online submission form, no fields are required; however, if you choose to make a public records request using this form, please note email addresses are public records. If you do not want your email address released in response to a public record request, please make your request in person or by postal mail.
